ADMISSION PROCESS

All placements for long-term care start with the Toronto Community Care Access Centre. (CCAC)
The steps in the Application process are as follows:
- The Client or his/her family contacts CCAC - Community Care Access Centre. If in the hospital the hospital Social Worker or discharge planner may help with this process.
- The CCAC placement coordinator will assess client needs to determine service requirements.
- The client and/or family complete the placement application.
- CCAC then determines eligibility for LTC Home Placement
- Applications are then forwarded to 3 homes of client's or family choice.
- The application will be reviewed by the home of choice to ensure client care needs can be managed. Once the application is approved or declined the home must notify CCAC within 5 business days of its decision. If approved, the Home will also designate which unit would be appropriate for admission.
- Once the application is approved applicant is placed on Home Wait list according to priority rating.
- The facility notifies CCAC in writing whenever there is a bed vacancy.
- With consideration of the client care needs, the bed is offered to the next applicant on wait list appropriate to requested accommodation and unit designation.
- Admission is scheduled.
Please note: As Nisbet Lodge has a Faith based designation, those wanting to live at Nisbet Lodge should indicate a preference for a Christian home when applying and selecting homes of choice. This will provide a slightly higher placement on the wait list.
